Levels of Care Offered:



LEVEL I - PERSONAL CARE


  • Protective Care and Watchful Oversight - including daily awareness of Resident's physical condition, normal activities & readiness to intervene in an emergency.
  • Rounds are every 2 hours 24 hours a day
  • Meals and afternoon and evening snacks
  • Weekly Laundry Service
  • Weekly Housekeeping services with a daily spot cleaning
  • Facility Activities Program
  • Medication Supervision or reminders
  • Supervision of Bath



LEVEL II - HANDS-ON MODERATE ASSISTANCE (in addition to Level I)


  • Mealtime monitoring and/or set up assistance
  • Rounds are every 1 hour 24 hours a day
  • Walking with resident to meals
  • Hands-on assistance with dressing, grooming or bathing
  • Reminders/supervision of toileting
  • Glucose monitoring and/or insulin administration
  • Vital signs, i.e. blood pressure taken more than 2x a month



LEVEL III - EXTENSIVE ASSISTANCE (in addition to Level I and Level II)


  • Special assistance with meals, including cutting up meats, use of condiments, supervision
  • Rounds are every 30 minutes 24 hours a day
  • Incontinence assistance or hygiene
  • More frequent well-being checks due to physical or cognitive impairment
  • Extensive medication management
  • Hands-on assistance with transfer or ambulation
  • Transfers requiring more than one staff member
  • Hands-on assistance with bathing, dressing and other personal care needs



LEVEL IV - COMPREHENSIVE ASSISTANCE (in addition to Level I, II, III)


  • Daily Bath with Grooming and Hygiene
  • Daily Housekeeping (M-F)
  • Daily Laundry (M-F)
  • Hourly Bowel and Bladder checks
  • Rounds are every 15 minutes



ALL RESIDENTS MUST BE ABLE TO WALK WITH A DEVICE OR PROPEL A WHEELCHAIR. THEY MUST BE ABLE TO GET TO SAFETY IN EMERGENCY.
